Record whether new leaves are smaller, pale, twisted, melting, pinholed, or clean and strong. That observation is more useful than only writing that a plant looks bad.
Plant notes should name specific species when possible. "Crypt leaves melting" and "stem tips pale" point to different timelines, while "plants bad" is too broad to guide action.
Use photos as plant data
Plant changes happen slowly enough that memory is unreliable. Take photos from the same front and side angles after trimming or dosing changes.
Comparing two weekly photos often reveals whether stems are compact, carpets are spreading, or older leaves are accumulating algae.
Do not judge plant success only from height. Compact growth, clean new leaves, stronger roots, pearling in high-tech tanks, and reduced algae on older leaves can all show improvement before the aquascape looks full.
Connect growth to maintenance events
Plant response can follow a trim, replanting, substrate disturbance, filter cleaning, CO2 adjustment, or missed dose. Those events belong in the same timeline as growth notes.
If melting starts after a rescape, the follow-up is different from slow decline over a month of inconsistent fertiliser.
New tanks often show plant transition problems while bacteria, substrate, and dosing routines settle. Mark the tank age so early instability is not confused with a mature-tank deficiency.

Balance light before adding more nutrients
More light raises demand for CO2 and nutrients. If light is long or intense while CO2 and dosing are inconsistent, algae and weak growth can appear together.

Flow can be a plant-growth factor. Dead spots collect debris and limit nutrient or CO2 distribution, while strong direct flow can damage delicate stems or expose rooted plants.
Track trimming as a growth input
Trimming changes light, flow, nutrient demand, and plant shape. It is not just a cleanup task; it is one of the main inputs in a planted aquarium.
Log heavy trims, replanting tops, removed mass, and shaded areas opened up. A growth burst or algae shift after trimming then has a clear explanation.
If a plant keeps failing, record whether it was moved, shaded, uprooted, eaten, or planted too deeply. Many repeated plant problems are placement problems rather than dosing problems.
Connect roots and substrate to the record
Root feeders can struggle even when water-column nutrients look acceptable. Disturbed substrate, exhausted root tabs, compacted areas, or newly moved plants can explain weak growth.
